Our office is conveniently located on 3140 S Rainbow Blvd Ste 401 Las Vegas, NV 89146. Our friendly and efficient Front Office Staff will assist you to arrange an appointment that is most convenient.

We are currently accepting new patients and offer both telehealth and in-office appointments.

Office Policies

Appointments/Cancellations

With the exception of serious emergencies, patients are expected to keep all scheduled appointments. If an appointment is missed, our office will automatically reschedule you for the next available opening to ensure continuity of care. We aim to be as accommodating as possible to meet your scheduling needs.

Telehealth Policy

To support high‑quality care and meet applicable federal regulations, insurance requirements, and pharmacy standards, all patients receiving care primarily via telehealth are required to complete at least one in‑office visit per year. This visit supports comprehensive evaluation, continuity of care, and safe prescribing practices. It is important to complete this annual in‑office visit to ensure this requirement is met and to avoid delays or interruptions in prescription refills.

Patient Conduct Policy

Our office is committed to providing a safe, respectful, and supportive environment for all patients and staff. To maintain this, we do not tolerate any disrespectful behavior including but not limited to yelling, threats, or harrassment. If a patient engages in such behavior, we may need to refuse services to ensure the well-being of everyone in our office. We appreciate your understanding and cooperation in helping us create a positive and caring environment.

Emergencies

Our office does not offer on-call or emergency services. All communications must occur during regular business hours. In the event of a medical or psychiatric emergency, please contact 911 or seek care at the nearest emergency department.

Our office accepts the following methods of payment for co-payments, deductibles and services that are not covered by health insurance:

We also offer the option for clients to do “self-pay” for our services. In response to current financial challenges, we have recently discounted our self-pay rates to better support our patients. If you prefer this option, please let our administrative staff know.

Discounted Self-Pay – New Patient Visit: $250
Discounted Self-Pay – Follow-Up Visits: $120
